My startup (incorporated in New Hampshire) will be changing its name soon. As I understand we will have to file Form 14 with the Secretary of State's office to do this.
However I'm a bit uncertain about exactly what I am supposed to fill in. Is FIRST the new company name and (Corporate name) on page 3 the current company name?
Secondly, what exactly should be filled in for SECOND? If we are doing nothing but changing the company name, can this be left blank?
